Registered Nurse – Home Health

Home Health. Full-time (36 hours/week) position. Monday, Tuesday, Wednesday, Friday; 8:00 am – 5:30 pm; Rotate call

The RN utilizes the nursing process to provide and direct quality patient care. The RN provides leadership when working with team members in maintaining standards of professional nursing practice. The RN participates with the interdisciplinary team to develop the plan of care and supports Home Health and hospice philosophies in caring for clients. The RN promotes health and wellness through education, clinical practice and home care for the ill and disabled.

Experience in acute care setting, home health, or hospice preferred. OASIS knowledge beneficial, knowledge of Medicare, Medicaid rules/regulations.

Graduate from an approved professional Nursing Program. Certification in Basic Life Support is required.

Director of Community Health

Community Health. Full-time (40 hrs/wk) position. Monday – Friday; 8:00 a.m. – 5:00 p.m.; Some call required

The Director of Community Health directs, assesses, plans, implements, and evaluates delivery of patient care to clients in the Community Health settings. Provides team leadership, development and evaluation of all department staff, programs and services, and maintains operational aspects in collaboration with administrative support services. Maintains all clinical, financial and statistical records and reports required in Community Health. Ensures health care services are available to all county residents by administering county, state and federal funding for public health, and assessing health needs within the community.

Minimum of three years in home care/hospice nursing experience recommended, part of which has been in a supervisory capacity. Has demonstrated ability in nursing practice and competence in management and team leadership. Considerable experience with grants and contracts preferred.

RN with Bachelor’s degree in nursing with current IA license

Fitness Monitor

Petersen Family Wellness Center. On-call position. Variable nights and weekends.

Provide supervision, guidance and assistance to Fitness Center participants; ensure the general appearance of the facility and equipment are clean and in good working order, and perform documentation and record keeping as required.

Knowledge and use of exercise equipment preferred.

Entry Desk Clerk

Petersen Family Wellness Center. On-Call Position. Variable nights and weekends.

Provide supervision, guidance and assistance in entering the Fitness Center. Assist with membership enrollment, POS and customer service to Fitness Center participants; ensure the general appearance of the facility, entry desk and vending areas are clean and in good working order, and perform documentation and record keeping as required.

Knowledge and use of exercise equipment preferred.

RN Clinical Manager

Home Health and Hospice. Part-time (32 hours/week) position. Monday, Tuesday, Thursday, Friday; 8:00 am – 4:30 pm; Rotate call.

The RN Clinical Manager directs, assesses, plans, implements, and evaluates delivery of patient care to clients in the Home Health and Hospice setting. The RN Clinical Manager will be responsible for making patient and personnel assignments, coordinating referrals and patient care and assuring patient needs are continually addressed. The RN Clinical Manager functions as a part of the Interdisciplinary Hospice Team. The RN Clinical Manager  assures adequate exchange of information and facilitates communication and interaction among all team members. The RN Clinical Manager assists with maintaining all clinical and statistical records and reports as required for Home Health and Hospice CMS participation.

Experience in home health/hospice preferred. Experience in caring for terminal patients and their families. Knowledge in the needs of clients who typically receive home health with diagnoses such as HF, diabetic, wounds, postoperative joint, postoperative cardiac, chronic, fragile and/or terminally ill patients and their families.

Graduate from an approved professional Nursing Program. BLS certification is required. Must possess valid Iowa driver’s license and maintain auto liability insurance.

Home Care Aide

Community Health Department. Part-time (24 hours/week) position. Monday through Friday (various days of week); 8:00 am – 4:30 pm; 24 hours; occasionally visits on weekends.

Under general supervision, the Home Care Aide provides a broad range of client related care services, social, environmental and personal care, in a home setting, to enhance the capacity of household members to attain or maintain independence and to maintain a healthful, safe environment. Provides personal care and light housekeeping in clients’ homes. Additional responsibilities as assigned.

Excellent customer service skills required. Valid Iowa driver’s license and proof of automobile liability insurance required.

CNA certification required. BLS required.

Environmental Services Staff – Part-time

Environmental Services Department. Part-time (20 hours/week) position. Monday – Friday; 5:00 pm – 9:00 pm or 6:00 pm – 10:00 pm. Rotating weekends/holidays

Assumes responsibility for overall cleaning of patient rooms, work areas and public areas according to standards of the hospital. He/she may be required to sort and/or transport laundry. Successful completion of the work on a regular basis would result in a clean, safe and attractive environment which is conducive to the recovery of ill patients and contributes positively to staff morale and the public relations of the hospital.

Training and experience not required. Hospital will do on-the-job training. Must know how to use common house cleaning devices to keep areas clean. Must know how to care for equipment, how to measure cleaning solutions, and be alert to hazards.

Completion of high school or equivalent preferred.

Environmental Services Staff – Full-time

Environmental Services Department. Full-time (40 hours/week) position. 5 days/wk; Rotating weekends/holidays; Schedule to be determined; Satellite Clinics included

Assumes responsibility for overall cleaning of patient rooms, work areas and public areas according to standards of the hospital. Performs various cleaning duties, sweeps and scrubs floors, cleans restroom facilities, handles trash and recycled items, dusts offices and performs other duties as assigned. Cleans patient and non-patient areas.

Training and experience not required. Hospital will do on-the-job training. Must know how to use common house cleaning devices to keep areas clean. Must know how to care for equipment, how to measure cleaning solutions, and be alert to hazards.

Completion of high school or equivalent preferred.

Nursing Supervisor

Medical-Surgical Department. On-Call position. Sunday-Saturday; Every third weekend.

Manages nursing activities in the delivery of quality, cost-effective patient care. He/she directs, coordinates and participates in the delivery of patient care. Responsibilities include, but are not limited to, staff performance appraisals, staff and scheduling, quality improvement, achievement of departmental goals, and membership on Nurse Management Council.

At least two years of clinical experience in Emergency Department as well as the Medical-Surgical area. Graduate from an approved professional Nursing Program. Current Iowa license. Certification in Basic Life Support and Advanced Cardiac Life.  

POD Nurse – Earling Clinic

Rural Health Clinic. Full-time position. CMA/LPN. 4 days/week; Monday, Wednesday, Friday in Earling = 8:00 a.m. – 4:00 p.m.  Tuesday in Earling = 8:00 a.m. – 12:00 p.m. and in the Harlan Clinic 1:00 p.m. – 8:00 p.m.

The Pod Clinic Nurse is of prime importance in helping the medical provider respond successfully to the increasing demands for his/her services. He/she is the essential link between the medical provider and his/her patients, professional associates, and equipment and medical suppliers. She/he delivers patient care through the nursing process of assessment, planning, implementation and evaluation.  

Graduate of an accredited school of nursing program possessing a current Iowa Nursing license or graduate of medical assistant training – possessing certification.  Must either be certified in CPR or obtain certification during the first three months of employment.
